Stats. § 349.166 desiring a permit to operate or conduct lawful gambling shall make application therefor on a form provided by the clerk. The application shall set forth the name and address of the organization which is to operate or conduct the lawful gambling, and the time and place where such lawful gambling is to be held. The organization shall also state affirmatively on the application that none of the officers or directors of the organization have been convicted of a felony or gross misdemeanor, or of violating any ordinance or law regulating gambling any place in the United States.
